When a bone or joint problem is an emergency

This page is from Dr. Subhash Ushakoyala, Specialist Orthopaedic Surgeon at Grand Infinity Medical Center in Al Muteena, Deira, Dubai. Some bone and joint problems cannot wait for a clinic appointment. If you see any of the signs below, call 998 for an ambulance or go to the nearest hospital emergency department now.

This website does not handle emergencies

Do not use the booking form, WhatsApp or a clinic call for an emergency. They are not an emergency service. Call 998 for an ambulance in the UAE, or go to the nearest emergency department.

Call an ambulance

Call 998 or go to an emergency department if

  • A bone is showing through the skin, or there is a deep wound over a possible break (an open fracture).
  • An arm or leg looks bent, twisted or the wrong shape after an injury.
  • A hand or foot below an injury is cold, pale or blue, or you cannot feel a pulse there.
  • You cannot feel or move the fingers or toes below an injury.
  • Someone has fallen and cannot stand or put weight on the leg, especially an older person after a fall onto the hip.
  • Pain after an injury, or under a cast or tight bandage, keeps getting worse and is far worse than you would expect.
  • A joint is suddenly hot, swollen and very painful and you have a fever or feel unwell.
  • Leg pain or swelling comes with shortness of breath or chest pain.

Why these signs matter

When a broken bone breaks the skin, germs can get into the bone. The wound needs cleaning in an operating theatre as soon as possible to lower the risk of infection.

Pain that is out of proportion to the injury, especially under a cast, can mean pressure is building inside the muscles (compartment syndrome). It cuts off blood flow and is a medical emergency. If you have a cast and this happens, loosen any bandage you can and get help straight away.

A hot, swollen joint with fever can be an infection inside the joint. It needs hospital treatment quickly to protect the joint.

Pain and swelling in the calf with breathlessness or chest pain can mean a blood clot has moved to the lungs. That is life-threatening.

While you wait for help

  1. Keep still

Do not move the injured limb or put weight on it. Support it with padding, such as folded clothing.

  2. Cover wounds

Cover an open wound with a clean cloth. Do not push a bone back in.

  3. Nothing to eat

Do not eat or drink, in case an operation is needed soon.

  4. Gather details

Have the address or a landmark ready for the call handler, and collect the person's medicines and any allergy information.

After the emergency

Once you have been treated in an emergency department, you can book a follow-up with Dr. Subhash. Bring the discharge letter, the scans and any operation notes.

Questions patients ask

What is the ambulance number in the UAE?

998. You can also go straight to the nearest hospital emergency department.

Can I message the clinic on WhatsApp in an emergency?

No. Messages and booking requests are not an emergency service. Call 998 or go to an emergency department.

Is a sprain an emergency?

Usually not, and you can book a clinic visit. It becomes urgent if the limb looks deformed, you cannot put any weight on it, or the foot or hand goes numb, cold or pale.

My cast feels too tight. What should I do?

If the pain keeps getting worse, or your fingers or toes go numb, pale or blue, get help now: call 998 or go to the nearest emergency department. If it is only uncomfortable, call the clinic the same day. Do not wait for your next appointment.
